PROCESS INDUSTRY ACCIDENTS

Lessons Learned the Hard Way and How to Avoid Them

PDH: 1 Hour Professional Development

 

About the Topic:

 

Using a collection of videos, photographs and stories, this presentation will cover lessons learned

 from a variety of process accidents - and how to avoid them. Topics will include:

·  Everyone needs training (yet they often don't get it or accept it)

·  People must follow procedures (yet they often don't, for a variety of reasons)

·  Even trained people make mistakes (and sometimes they do really stupid things)

·  Some people don't know what they don't know (and ignorance is definitely not bliss)

·  We're not as immune or indestructible as we may think

·  We can't foresee every possible hazardous scenario

·  Reuse of software has not always successful

·  Near misses are often not followed up

·  The past is often ignored (and history definitely repeats itself)

·  The various personnel functional safety certification/certificate programs available

(e.g., CFSE, TUV & ISA) and the differences between them

·  Member of the ISA 84 standard committee (on safety instrumented systems)

·  Member of the ISA 101 standard committee (on human machine interfaces)

·  Developer and instructor of ISA courses on safety systems

·  Author of two ISA textbooks ("Safety Instrumented Systems Design, Analysis &

Justification" and "Sell More Through Effective Technical Presentations"),

two chapters in other books, and over two dozen published articles

·  Developer of the first commercial safety system modeling software

·  B.S. degree in Mechanical Engineering from Illinois Institute of Technology
